Request new/changed Medical ID, email account or network file space

New faculty, staff and students

Just as they need a physical ID badge to enter campus facilities, new faculty, staff and students will need a Medical network ID to access the campus information network and associated services like email. 

Staff will receive their Medical ID's when they attend the New Employee Orientation, which is administered by Professional Development and Training. Medical ID's for faculty and students can be requested by emailing the  Help Desk. A supervisor should send the email from his/her Exchange email account, indicating each person's full name, ID number (C-number), position/title, and a contact telephone number.

If the person should have access to your department's/organization's shared file directories, please indicate these directories in the email request for a Medical ID.

You may need to help your supervisees with the steps of obtaining other virtual IDs.

Requests are subject to confirmation/verification.

Changes to Medical IDs, email, file space or other network privileges

Send an email to the Help Desk.  Provide the person's C-number and name, Medical ID, and any other information relevant to the change requested.

Departing employees & Clearances

An IDR must be submitted to Medical Information Technology to terminate a person's access to the network (and to associated resources like email) and to discontinue billing for any chargeable services.

If there is a need to coordinate the timing of access termination, e.g, because of security issues, please contact our Information Security group.
