Information security incidents

You should report:

  • Loss or theft of sensitive organizational data, including but not limited to patient health care records, student educational records, customer or organizational financial data.
  • Loss or theft of any computer, laptop, portable device or storage media containing such sensitive information, whether the device belongs to the organization or to you.
  • Any compromise -- or suspected compromise -- of a password, access token or other mechanism used to gain access to information.
  • Any technical problems with hardware, software or networks that you suspect could be security-related.

If in doubt, report.  It's better to err on the side of safety.
